Training Regulations

Training Regulations – Organized by SHT Anna Jaszczewska-Domagała, with its registered office in Warsaw 01-885, 36 Fontany St., apt. 2, NIP: 1180288309, effective from June 1, 2016.

§ 1 Training Organizer

  1. The trainings are organized by the company SHT Anna Jaszczewska-Domagała with its registered office in Warsaw 01-885, 36 Fontany St., apt. 2, NIP: 1180288309.

§ 2 Trainings

  1. The Organizer conducts training within the scope and on the dates specified in the training offer published on the following websites: www.szkolenia-dla-dentystow.pl, www.szkolenia-dla-lekarzy.pl, www.shtraining.pl
  2. The Organizer appoints trainers with appropriate substantive preparation, experience, and competence to conduct the training.
  3. Classes take place in teaching rooms equipped with appropriate multimedia equipment and auxiliary facilities.
  4. Within the price of the training, each participant receives the necessary training materials.
  5. Upon completion of the training, the participant receives a certificate of completion.
  6. In order to guarantee the highest quality of training services, training participants will be asked to fill out evaluation questionnaires assessing the organization, training methodology, and the conducting trainers.

§ 3 Conditions of Participation in the Training

  1. The number of places for the training is limited; the order of registration decides.
  2. The basis for participation in the training is reading these Training Regulations and submitting a completed application for the selected training via the online registration form available on the website, or through another acceptable form of application, e.g., by phone (including via SMS message) to the phone number provided on the websites, or by email to the email addresses provided on the websites.
  3. Submitting the online registration form, phone registration (including via SMS), or email registration is equivalent to a payment obligation on the part of the Ordering Party. Registering in any of the above-mentioned forms is equivalent to accepting the provisions of these Training Regulations.
  4. Applications are accepted at the latest up to 2 working days before the start of the training (the date of receipt of the application by the Organizer counts).
  5. The training participant is obliged to request educational points 14 days before the course. In such a case, an additional fee of PLN 250 is charged. Unreported courses do not receive points.
  6. After the deadline of 2 working days before the start of the training, registration of participation is permissible only upon prior telephone agreement with the Organizer.
  7. After receiving the application for the training, regardless of the form of submission, the Organizer will send a confirmation of participation in the training by email or via SMS message.
  8. The Organizer reserves the right to change the number and types of procedures during the courses.

§ 4 Payment Terms

  1. The amount of the training fee is in accordance with the current schedule and training price list published on the website.
  2. Training prices include: the cost of the training, a full set of training materials, and a certificate. They do not include the costs of accommodation, travel, parking, catering, and overnight stays.
  3. Payment for the trainings must be made by bank transfer to the bank account specified in the issued invoice.
  4. Payments for the training must be made by transfer within the time limit indicated in the invoice.
  5. For the collected payment, the Organizer will issue a VAT invoice for participation in the training.
  6. Training prices are provided in Polish Zlotys (PLN).
  7. The training Organizer is not responsible for the failure to receive funding from Financing Institutions.
  8. In the event of failure to receive funding for professional training in the amount of at least 70% of the training cost, resignation for other reasons, failure to receive funding, or cancellation of the concluded agreement with the fund operator, the Entrepreneur undertakes to cover the training costs in the amount of 100% (i.e., the full value of the training for the development service Provider) within 7 days from the end date of the training.
  9. By signing up for the training, the buyer hereby authorizes the Organizer to issue a VAT invoice without the Purchaser’s signature.
  10. By registering for the training, the Ordering Party agrees to receive a VAT invoice in electronic form, which will be forwarded to the email address of the Ordering Party.

§ 5 Changes to the Date and Venue of the Training

  1. The Organizer reserves the right to change the date or venue of the training, as well as to cancel the training up to 3 days before its planned start date, due to organizational reasons or unforeseen circumstances.
  2. The Organizer also reserves the right to appoint an equivalent replacement in the event that the trainer cannot conduct the training due to unforeseen circumstances.
  3. Participants registered for the training, if it does not take place as planned, will be informed about this fact by email or telephone.
  4. Information regarding the cancellation of the training is sent to the email address indicated in the training participation application.
  5. In the event that the training does not take place for reasons attributable to the Organizer, the payments made will be refunded within 10 working days from the planned date of completion of the training.
  6. The Organizer will also inform via email about the scheduling of the next training date.
  7. The Ordering Party cannot claim compensation for damages or lost profits resulting from the cancellation or postponement of the training date.

§ 6 Resignation from Training

  1. Resignation from participation in the training must be reported to the Organizer by email. The Organizer will confirm the receipt of the resignation from the training.
  2. In the event of resignation from participation up to 21 working days before the start date of the training, and if the training has already been paid for, the paid amount will be refunded in full to the bank account indicated by the Ordering Party.
  3. Resignation from participation not reported 21 working days before the start of the training does not release from payment. In such a case, the Ordering Party who is an entrepreneur undertakes to bear a fee in the amount of 100% of the training price.
  4. Resignation from participation not reported 21 working days before the start of the training made by an Ordering Party who is a consumer does not release from payment. In such a situation, the Ordering Party who is a consumer is obliged to cover the costs incurred by the organizer (up to the moment of resignation by the Ordering Party) for organizing the training.
  5. Full or partial absence from the training of a participant whose training has been paid for does not constitute a basis for a claim for a refund of all or part of the training fee against the Organizer.
  6. Resignation is not possible in the case of a training whose date has been moved at the participant’s request.

§ 7 Post-training Support

  1. After completing the training, the Ordering Party is entitled to 30 days of post-training support, during which the training participant has the opportunity to contact the trainer in order to deepen their knowledge regarding the training in which they participated.
  2. An Ordering Party wishing to submit organizational and training remarks is obliged to do so within a period not exceeding 7 days from the end of the training.

§ 8 Personal Data

  1. The controller of personal data in connection with the use of services remains, in particular, the company SHT based on an entry in the CEIDG, NIP 1180288309, REGON 012388064, with a permanent place of business in Warsaw, postal code 01-885, 36 Antoniego Fontany St., apt. 2.
  2. Personal data are processed for the purpose of implementing the training service, contacting on matters concerning the training, debt collection, for archiving purposes, and ensuring accountability based on provisions of law.
  3. Personal data may be transferred to external companies solely for the purpose of implementing the training service.
  4. Personal data will be processed for the period necessary to perform the service. In order to demonstrate accountability, personal data will be stored for the period during which the Organizer is obliged to store them in electronic or paper form, to document compliance with requirements resulting from legal provisions and to enable inspections by authorized entities.
  5. Every person whose data are processed as part of the performance of the training service has the right to access the content of their data and the right to rectify, erase, restrict processing, the right to data portability, the right to object, as well as the right to lodge a complaint with a supervisory authority if they consider that the processing of their personal data violates legal provisions.
  6. Providing data is voluntary but necessary to be entered on the list of training participants and to issue an Invoice for the training.

§ 9 Final Provisions

In matters not regulated by these regulations, the provisions of the Civil Code shall apply.

These regulations apply from June 1, 2016, until further notice.

The Organizer reserves the right to introduce changes to the regulations. Each User will be informed about the content of the changes to the regulations via email.
